Q. How do I apply for credit?
A. Critique and attestation are included with your program. You may either fax, mail or email your critique and attestation. Your certificate will returned to you via secure PDF email and regular USPS.
Please note if you are paying with a personal check: To ensure timely processing of your attestation for credit, please remit before applying.
Q. When will I receive my certificate?
A. Certificates are processed upon arrival in our office.
Q. How do you verify my participation?
A. We do not collect any outcomes from your participation. Your critique and attestation for participation are accepted with your signature and date.
Q. How do I obtain a registration code or Login? How do I register for Mammography only?
A. For the 2006 version of mammography Practice Based Education, the registration process has changed. If you arrive at the registrations screen, make sure that you have completed step 2 of the installation process ( Close Practice Based Education program and reinsert the CD ROM - follow the on screen instructions. )
Q: Login: for PET/CT only?
A. Your login can be found on your invoice and on the instructions that came with your disk. Note: when attempting to login, you will need to be connected to the Internet at the time that you enter your login and password. Also, if you are participating from a CD-ROM version, the CD must be loaded in the drive prior to login.
